Our Gurnee office is  located at 351 S. Greenleaf Street Suite C, Park City, IL 60085. The practice is conveniently located in the northern suburbs near the Tri-State Tollway and Skokie Highway. We have an on-site parking lot for the convenience of patients and their families.

Our Chicago office is located at 401 W. Ontario Street Suite 240, Chicago, IL 60654.  Take advantage of ample parking in the SP+ Parking station on the next block. If using public transportation you can take the number 37 bus to the Orleans & Erie station which is a two-minute walk from our office.
